2011-2012 Patient Survey

In 2011 our practice aims to set up a Patient Participation Group to help us improve the services we provide for our patients.  If you would like to join our Patient Participation Group please contact the surgery on 01274 521111 and speak to Nisa Iqbal or Sharon Hirst.  If you are unable to attend at the surgery but have access to emails, you can still take part.....we have already recruited several patients who have agreed to be involved by email contact only.  We would welcome your input and help in this project and look forward to hearing your views about the services we provide.

In addition to our Patient Participation Group Parkside Medical Practice have now uploaded our new Patient Survey onto Survey Monkey at:  https://www.surveymonkey.com/s/7QQVC6P  Please take a few minutes of your time (no more than10 minutes) to complete the survey - your views and comments will be very much appreciated by the practice and we will feed back the results towards the end of January, 2012.  PLEASE HELP US TO IMPROVE OUR SERVICES AND THE WAY WE WORK.

New Pregnancy Testing and Contraception Service

We are now offering the following services for our patients:

Pregnancy Testing Service:

This will enable patients to receive results on the same day, rather than waiting for test results to come back from the laboratories.  If you are registered at our practice,  would like a pregnancy test and your last menstrual period was more than 4 weeks ago, bring a urine sample into the surgery (or alternatively ask for a bottle at our reception desk).  As our receptionist for further details.  Service available Mon, Tues Thurs and Friday 8.30am - 2.30pm and Wednesday 8.30am to 11.30am only.

Contraception Service:

 We can now offer a full Contraception Service to our patients.  This includes information, advice and counselling on  ALL methods of Contraception, including Coil Fitting, Implants, Pill and Depot Injections etc.  Please contact the surgery to make an appointment with  Dr. J. A. Azam or Dr. S. R. Azam.

We are always looking for ways to improve our patient services, if you have any comments or suggestions about new or existing services in our practice that you would like to pass on, please complete a comment/suggestion form at our main reception desk or alternatively ask to speak to Duty Manager Sharon Hirst.

NHS National Patient Survey
