Summary of Patient representative group, survey results and action plan

PATIENT REPRESENTATIVE GROUP (PRG)

 

 

Members of the PRG:

We have a multi-ethnic population and efforts were made to identify the different ethnic groups to be represented in the patient reference group.

 

The invitation for participation was offered to all patients, who regularly attend the surgery by notices in the waiting room and personal invitation.

 

PATIENT SURVEY

 

Priority issues such as access, waiting times, patient care, etc were identified and a questionnaire was handed out to the patients attending the surgery.

 

The responses were collected and the results were calculated using the survey monkey tool. The results of the survey were used to identify the issues that needed to be addressed at the PRG meeting.

 

 

 PRG MEETING

 

The first PRG meeting was held on 15th of September 2011 and was attended by the members of the group, the practice manager and the secretary.

 

During the meeting the results of the patient survey was discussed and the issues raised by the PRG was noted by the practice manager, who allowed time for comments from the PRG.

 

The secretary took minutes to ensure accuracy of the contents of the meeting. Priority issues were discussed in order to reach an agreement.

 

ACTION PLANS

 

Action plans were made for priority issues, it includes:

 

·        The access hours of the surgery including the reception and surgery hours to be displayed on the notice board in the waiting area.

 

·        Letters to be sent to patients who fall under the vulnerable category to attend for flu vaccination.

 

·        Letters to be sent to patients to attend for chronic disease management for those whose targets are not achieved.

 

·        Letters to be sent to the local authorities regarding better parking facilities for the disabled.

 

 

1.    The result of the practice flu uptake for over 65’s for the year 2011/2012 is 55% as against 69%- the PCT average. We have informed the PCT of our progress in the flu vaccine uptake.

PATIENT PARTICIPATION GROUP MEETING ON

7th OF DECEMBER 2011

The matters that were discussed in the PRG meeting were:

 

FLU UPTAKE

 

This was a serious issue which was discussed in the meeting. Some patients in the vulnerable group i.e over 65 years, chronic heart and lung diseases etc, do not understand the need to take up flu vaccination.

 

Some patients have needle phobia and are reluctant to attend.

 

Despite several notices, telephone callas and letters, the uptake in our practice remains low at 40%. The cluster average is 52%.

 

The patient group advised the practice to try and encourage the patient to attend for flu vaccinations.

 

CHRONIC DISEASE MANAGEMENT

 

Some patients do not have an insight into their illness. E. g: diabetic or hypertension. Once they have advised control, they think they are cured and stop taking medication. Some take it randomly.

 

Compliance issued has been reviewed at consultations.

 

Also they need to have regular check for chronic disease management with nurses has become an issue. DNA’s at there clinic, despite text messages and phone calls.

 

PRG advised the surgery to continue seeking co-operation of the patients, so that they can benefit with their health control. Public education leaflets are in surgery and patients are encouraged to read them.

 

 

PARKING

 

This issue has not yet been resolved. The practice has written to authorities about parking problem in front of the surgery but so far no responses are forthcoming.

 

Practice will continue to draw attention of the local parking authorities on this.

 

OBESITY

 

This links up with chronic disease management. To avoid complications, lifestyle advice, exercise and dietician referrals are offered by the practice. Patients need to take them more seriously. In order to prevent complications of their health problem.

REPORT FOLLOWING THE PATIENT GROUP MEETING ON THE 15TH OF SEPTEMBER 2011.

 

 

The PRG meeting was attended by the following patients:

  6. Mr. Paramaswamy Mahalingam

The matters that were discussed in the PRG meeting were:

 

  1. Access
  2. Patient Care
  3. Parking Facilities
  4. Pharmacy Services
  5. Texting Services
  6. Support Group.

 

ACCESS:

 

With reference to access the surgery is open from 8 am to 6: 30 pm, Monday to Friday. On Mondays we have the extended hours until 7 45pm. Notices are displayed to ensure that the patients are aware of these opening times.

 

Waiting times was an issue that was raised in the PRG meeting.

 

There was confusion about the waiting times during the walk in clinic on Tuesdays and Thursday mornings. These clinics are available for patients who need urgent appointments. One doctor will be running this emergency clinic. The other doctors and nurses will be seeing booked appointments. Unfortunately there is some confusion from the patients who are on the emergency list that they are not called up for their appointments on time. They confuse their waiting times with the patients who are with pre-booked appointments.

 

After discussion with the patient reference group a notice explaining this has been put on the notice boards in the waiting room.

 

PATIENT CARE:

 

The patient participating group indicated that the primary care staff treated them with respect and the medical care offered was excellent. There were no complaints on the care of the patients.

 

 

PARKING FACILITES

 

Parking for disabled patient s was raised by the PRG. Though we have two parking bays on top of Mornington road which are for 20 minutes maximum parking most drivers leave their cars permanently.

 

The single yellow line in front of the surgery is also blocked by drivers who park inconsistently blocking access to the surgery.

 

This has created difficulties especially for patients with disabilities.

 

We have raised the issue of parking with the authorities in the past but no action has taken place. The PRG requested us to double our efforts to request disability parking bays in front of the surgery from the authorities.

 

The practice is making a formal application for this.

 

PHARMACY SERVICES

 

Some patients are not aware of the prescription collecting services offered by the local pharmacies. This information is being provided on the notice board, practice leaflet and the JX board in the waiting room.

 

TEXTING SERVICE

 

Patients were happy with the newly introduced texting service and wanted it to be continued.

 

SUPPORT GROUP

 

The PRG requested more information for chronic disease management eg: Diabetes. The practice is producing more leaflets to be made available to patients.
