'Together we will make a difference'

We are a group practice committed to providing the best possible medical care for all our patients and to the NHS. Our clinical team currently consists of 12 doctors, one nurse practitioner, one lead practice nurse, two specialist practitioner practice nurse's, four practice nurses, one assistant practitioner and four health care assistants. We have friendly and helpful reception staff and we are backed up by an efficient and professional administration team.

The practice is situated in Thornton Heath, three miles from Croydon, Surrey. We serve an urban area and look after a population of approximately 14,400 patients.

The surgery was opened in 1996 back on its original site. The building has been refurbished to a high standard and designed to provide efficient and comfortable facilities for all our patients with an informative waiting room with touch screen check-in and modern consulting rooms.

October 2010 saw the opening of the Beulah Wing. The new wing provides an additional six state-of-the-art consulting rooms, a fully equipped minor operations suite and a conference room also equipped with the latest audio and visual technology. The Beulah Wing will support facilities and services provided at the practice.

PATIENT ACCESS - BOOK APPOINTMENTS AND ORDER REPEAT MEDICATION ONLINE!

pa

You are able to book an appointment online, request repeat medications and view your medical records via the patient access website or patient access mobile app. With patient access, you can now access your local GP services at home, work or on the move - wherever you can connect to the internet. What’s more, because patient access is a 24 hour online service you can do this in your own time, day or night. Please ask at reception to register to use this service.

For more information please vist www.patient.co.uk

From 1st March 2015 patients are able to access part of their medical records online. This includes medications, immunisations, allergies, sensitivities and Detailed Coded Record (latter from 31 March 2016). Please ask for a form at reception or download the attached form. You will need to provide two forms of ID, at least one must include a photograph and the other bank related (utility bills are not acceptable). If you are well known to the practice a member of staff may be able to voucher for you should you not have the required ID. There are strict controls about providing access as your data is obviously very sensitive and personal. Please do not take offence if for whatever reason we do not grant you access. At this current moment in time access will only be granted to patients 18 years and older.

Named Accountable GP

Since April 2015, all NHS GP Practices have been required to allocate a Named Accountable GP for every patient on their list, including children. This is required by the Government under the terms of the latest GP contract.

If you registered with us prior to 3rd July 2010 your Named Accountable GP is the GP you registered with at the time. After this date you would have been given a name of a GP upon registration, referred to as your usual GP. This is your Named Accountable GP.

If you wish to confirm who is your Named Accountable GP please ask one of our staff at the Reception desk when you next visit the surgery or the clinician at your next appointment. Please do not phone as this could lead to excessive telephone queues and may cause delay to those who need urgent treatment.

Having a named GP does not prevent you seeing any other doctor in the practice. Please note this is not a 24 hour service. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor. Where a patient expresses a preference as to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request.

If you have any queries about this please do not hesitate to contact the surgery.
