Summary Care Record Project

The practice is participating in the Summary Care Record Project and will be amongst the first practices in the area to create Summary Care Records.

The Summary Care Record will be used in emergency care settings, like the Walk in Centre, or A&E. The date for record creation in this practice will commence on 27th September 2011.

The record will contain Medication, Allergies, Adverse Reactions.

In future it may be added to with further information such as diagnosis, End of Life Care Plan and information concerning blood transfusion preferences.

Should you feel you do not wish to have a Summary Care Record created, please speak to one of our reception staff who will advise you of what you must do.

In addition to Summary Care Record creation, you can apply for an Advanced Healthspace account. This will allow you to view your own Summary Care Record for an internet enabled computer.

It can be useful when you go on holiday outside the country; if you have to visit a doctor or hospital.

Please speak to one of our reception staff for information on how you can apply to an Advanced Healthspace account or refer to the posters dotted around the practice.

Oral glucose tolerance testing

Oral glucose tolerance testing will be available in surgery from mid-June. Patients will be referred by GPs for the test which will help in the diagnosis of diabetes.
