Change of patient's personal details

If you change your name, address or contact details (such as your telephone number or email address), please let us know as soon as possible so that we can update our records. If you are receiving treatment at a hospital it might also be a good idea to inform them of any changes so that both our records are consistent when we communicate with them.

  • Be on time for your appointment.
  • Tell us if you need to cancel.
  • Call for a home visit or urgent appointment before 10am.
  • Ring for test results after 1pm.
  • Be courteous to all staff at all time.

Who has access to patient information

We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can provide the best possible care.  This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care.  You have the right to withdraw your consent to information being shared.

Policy on violent or abusive patients

We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, we may exercise our right to take action to have them removed, immediately if necessary, from our list of patients.

Catchment area

If you live within our practice area you will be able to register with the practice (details are available at reception).  If you move out of the area you will probably be asked to register with a new GP.  We feel it is important our patients live close to the practice in case you are seriously unwell and we need to get to you as quickly as possible.

How to get your test results

The practice has a strict policy regarding confidentiality and data protection.  We will only release test results to the person to whom they relate unless that person has given prior permission in writing for the release of the results or they are not capable of understanding the results.

If investigations are arranged by either the doctor or a nurse, please ensure that you understand how to check results.  An enquiry to reception will tell you if the result is available and whether or not you need to see the doctor again.

Please telephone for your results after 2.00pm.  This allows time for the results to be processed and checked by the doctor.  It helps enormously, if when requesting results you can be as specific as possible about what the test was for and when it was done.  Samples for the hospital should be brought to reception before 11am weekdays only.
