Avondale & Holme Hall Patient Survey Results 2012




The Practice has recently completed the first of a new patient survey at both Avondale & Holme Hall Surgery sites.  This is the first of this style and will be used as a benchmark for improvement and to compare our performance year on year. The results will be published on our website in March 2012 for our patients to view and comment.


The results will be used as a basis to identify and plan training for staff and GPs. It will also be shared with the Patient Group to ensure appropriate action plans are in place in response to the results, ensuring that we are focussed on achieving improvements for our patients.


All patient’s comments and feedback to the survey results are welcomed and appreciated.





Patients were surveyed during a 3 week period in February 2012.


Copies of the questionnaires have been used to provide the results of the survey, providing you with the opportunity to view the questions and responses.  There are two types of survey:



  1. Surgery Questionnaire  


A generalized survey, gleaning patient’s opinions and perceptions.


  1. GP & Nurse Questionnaire


A survey that focus’s on clinical care and management of your health.




In total 1,100 questionnaires were issued to patients with a response received from

549;   Approximately 50% .


The numbers of questionnaires issued varied for each of the surveys:


Of the 300 issued for the  Surgery Survey 88 were returned.                      29%

Of the 350 issued for the  GP’s  Survey 176 were returned.                         50%

Of the 450 issued  for the Nursing Team Survey 285 were returned.          63%

New stop smoking service

New Stop Smoking Service starts on Friday 4th November 2011 at the Sure Start Centre Wardgate Way Chesterfield.  This will be running on a Friday morning initially for 7 weeks.  If you are interested in stopping smoking then you can ring and register on 0800 085 2299

Travel form




In order to ensure that you are adequately protected against some diseases when you travel abroad, please fill in this questionnaire as early as possible.  Once the questionnaire is completed, you will need to ring the surgery one week later.  The Practice Nurse will have checked your medical records and will indicate on the form if you require any travel vaccinations prior to you holiday.  It is VERY important that you have any recommended vaccinations at least 4 weeks prior to your holiday.  Please be aware that there is sometimes a 3-4 week wait to make an appointment with the Practice Nurse.  It is your responsibility to ensure that you are adequately protected and that you have allowed yourself enough time to obtain an appointment for any vaccinations you may need.  The Nurse will only telephone you if your date of departure is less than 2 weeks of you returning your form.

Patient Participation Group

We propose to start a patient participation group at the surgery the purpose of this group is:

 To give practice staff and patients the opportunity to discuss topics of mutual interest in their Practice.

To provide the means for patients to make positive suggestions about the Practice and their own healthcare.

 To encourage health education activities within the Practice.

To develop self-help projects to meet the need of fellow patients.

To involve further patients from the wider population.

Avondale Surgery is on the move!

Patient email addresses

The surgery are currently asking all patients who have email access and would like to be contacted by email to inform us of their email addresses. This will allow us to send appointments, leaflets and deal with general enquiries in this way.

No email exchange is ever 100 per cent secure so patients need to inform us by email that they are happy to take part in this programme.

Beware Parking restrictions on Avondale Road

As from Monday 12th April 2010 Council parking restrictions came into force on Avondale Road. There are a limited amount of parking bays that can be used. Parking is restricted to 2 hours and no return within 1 hour. We have limited parking in our own car
