Overview

SYSTMONLINE  

The practice uses a clinical system called SystmOne.

This is a service we offer where patients can order prescriptions, book appointments and cancel appointments or check their appointment online at any time.

To access this you need to attend the surgery with a form of photographic Identification with you and ask for a letter which will include your individual username and password (please note if you do not provide photographic Identification we will be unable to provide you with this).

Unfortunately to ensure patient confidentiality we will not be able to do this initial step over the telephone.

Bowel Cancer Screening Campaign

The Practice is currently working with the Bowel Cancer Screening team. All patients (male and female) aged 60 - 74 years will be sent a screening kit to participate in the screening.

If you have any queries regarding this screening or wish to discussion anything about the screening programme please contact the surgery and request to speak to the Practice Champion (Kelly)

Extended Hours Access Scheme

The Practice offers early morning pre bookable appointments on a Thursday morning with the GP and Practice Nurse.

We offer appointments with the GP and Practice Nurse from 7.30am, to book one of these appointments please contact the Practice on 0113 2705620

Online Access for patients

systmoneYou may already be aware that you can book or cancel your appointments online from anywhere with an internet connection. 

Now you can also order your prescription online too.

If you take regular medication you can use SystmOne Online and request any items that you will be soon be running out of. You will only be able to request your medication 7 days prior to running out.

If some of the items are available but not all then please call the surgery and we will endeavor to make all your repeat medication available.

You can also view your detailed coded clinical record if you request this on line.

To register for online access please present at reception to speak to a member of the reception staff to complete the appropriate request form and please bring with you a form of photographic identification (for example a passport or driving licence)

Patient Participation Group (PPG) is made of practice staff and patients that are representative of the practice population. The main aim of the PPG is to ensure that patients are involved in decisions about the range and quality of services provided and, over time, commissioned by the practice.

Cervical Screening Awareness

All women aged between 25 and 64 are invited for cervical screening.

Women aged between 25 and 49 are invited for testing every three years, and women aged between 50 and 64 are invited every five years.

The Practice will, once you are due for a Cervical Smear, contact you by letter to make an appointment for the test to be performed.

Changes to Practice Boundary Area

Up until November 2012 we registered patients in Leeds 10 and parts of Leeds 11.

We have now made changes to our Practice Boundary area and we now accept new patients living in Leeds 26 - incluidng Rothwell, Carlton and Woodlesford.
