THE NHS DATABASE and THE HAMPSHIRE HEALTH RECORD DATABASE
The Oaklands Practice will not upload your medical details to the NHS Database (or “Summary Care Record”) without your active and explicit consent.
We will not make your data – or your children’s data - accessible in this way until you say so – until you opt in.
It’s your data, nobody else’s.
You do not need to opt out to prevent your medical records being uploaded.
However, you may still wish to register your opt out with us.
Your records cannot be uploaded if you have opted out, no matter what pressure a government or PCT may apply to GPs and despite any attempts to coerce or force GPs to upload patients’ records.
Opting out will also ensure that your wishes are recorded in your notes and clearly visible should you have need to register with a new GP surgery.
You can opt in, or back in, to the NHS Database at any time in the future.

Opt out forms are also available at reception, in the lobby and in the waiting room.
The opt out form can be used to protect your children as well.
This will ensure that their medical records are not automatically uploaded to the NHS Database.
You do not have to make an appointment with your GP just to opt out.
If you prefer, just state your wish to opt out to reception or to your GP the next time that you see him/her.

The Oaklands Practice will not upload your medical details to the Hampshire Health Record Database (the local version of the NHS Database) without your active and explicit consent.
If you are registered at the Oaklands Practice you do not need to opt out of the Hampshire Health Record.
We will not upload your records until you say so – until you opt in.

MEDICAL RECORDS LOST AND STOLEN

There have been a number of reports in the media recently regarding NHS medical records held on laptops being lost or stolen.
The Oaklands Practice does NOT hold any medical records on laptops, USB sticks or other portable devices
