Hi have tried in vain to obtain a card for myself, husband and child. Firstly your online application form is not working. For spouses and dependent children you cannot input the NHS number or NI number as it says invalid. Then the telephone system persistently gets our details wrong and I cannot get the correct details over. Then I went to our main post office in Bakewell for a form - and they no long have them. What would you advise - I am happy to write to DoH if needed. This is an disgraceful non service.
